Dr Liz’s Story

Elizabeth Landsverk, MD (Dr Liz) has practiced medicine for over twenty years and founded ElderConsult Geriatric Medicine, a house-call practice, to address the challenging medical and behavioral issues often faced by older patients and their families.

She has been board-certified in Internal Medicine, Geriatric Medicine and Palliative Care Medicine. Dr Liz was an Assistant Professor of Medicine at University of California, San Francisco, as well as consultant to the San Francisco Elder Abuse Forensics Center. She graduated from Stanford University, and trained at Cambridge Hospital, Harvard University and Mt. Sinai Medical School. She is currently medical director at Silverado Dementia Community.

As a geriatrician, Dr Liz collaborates with local physicians to address the complicated needs of vulnerable elders to alleviate pain, agitation and discomfort through use of the latest geriatric and palliative care techniques.

Dr Liz works with a vetted team of experts to provide top-notch care. Together, they have decades of experience and deep knowledge of eldercare.

Colleen Hagan

“I helped care for my grandmother when she was in a nursing home. It opened my eyes to the struggles families face while trying to care for a loved one. I want to continue to help and be a positive force for good.”

Colleen Hagan joined the DLG team with a diverse background and nearly twenty years of experience in homecare, five years in clinical research and five years in education. She earned her bachelor’s degree from Stone Hill College and studied public health at Harvard University. With her extensive experience in home health, she has worked in a number of management and operational roles. Colleen is the Director of Client Care and Training at First Light Homecare, providing in-home care for adults, including elders and those living with dementia. In her free time, Colleen loves long walks on the beach and hugging every dog she sees!

Yana Rudakov

Yana Rudakov is a distinguished Certified Care Manager and seasoned senior care expert with over two decades of professional experience. Recognized as a leading geriatric care specialist on the Peninsula, Yana’s expertise lies in dementia and Alzheimer’s care. Passionate about planning and coordinating the care of seniors and adults with impairments, she strives to enhance their quality of life and maintain independence. Yana is dedicated to educating clients and families on aging resources, fostering open communication, and ensuring the best long-term care outcomes.

 

 

Daizel Gasperian

“It’s very gratifying when I know I made a difference in someone’s life…even for a brief moment making someone smile…that brief moment made a difference.”

Daizel graduated from the University of California at Irvine with a bachelor’s degree in biology, and University of Southern California with a master’s in business administration.  She started her career in long-term care working in skilled nursing facilities, becoming a licensed administrator in 1997.  She eventually transitioned to assisted living, specializing in dementia care. Daizel rose through the ranks in operations and has served in various leadership positions, including Senior Vice President of Operations.  She is a certified dementia practitioner and geriatric care manager with over 30 years of experience.

Elizabeth Landsverk, MD is the founder of ElderConsult Geriatric Medicine, a house-call practice. This practice eventually evolved into Dr Liz Geriatrics — an online community, education and support service, as well as a telemedicine practice.

Dr Liz has been board certified in internal medicine, geriatric medicine and palliative care medicine and has served as an assistant professor of medicine at University of California San Francisco. She’s also served as a consultant for the San Francisco Elder Abuse Forensics Center. Through her house-call practice, Dr Liz has helped thousands of elders. As an expert in managing dementia agitation without sedation, she’s kept many patients deemed “unplaceable” from eviction or languishing in hospitals.

With over fifty million adults 65+ in the United States and only about seven thousand geriatricians, Dr Liz is focused on spreading her knowledge to the widest audience possible. She and her team provide direct advice and support to families, as well as educating healthcare workers, lawyers, guardians, and others who work professionally with elders.

Yana is both a registered nurse and Certified Care Manager with over 25 years of experience. She has a master’s degree in nursing, marriage and family therapy, and gerontology. In addition to serving as a member of our medical care team, Yana will be leading one of our upcoming support groups. Because of her diverse experience, Yana is uniquely qualified to look over the metaphorical “fence” between care management and nursing.
